CASE N0. 20205001 & 20205002 NOTICE OF HEARING ON PETITION FOR ADOPTION To: Brooke Jacks fka Brooke Elaine Sparks, whose whereabouts are unknown and cannot with reasonable diligence, be ascertained. You are hereby notified that on January 3, 2020, petitions for the adoptions of Brooklyn Alecce Jacks-Sparks, a child who was born on April 12, 2010 and Matthew Wayne Jacks-Sparks, a child who was born on February 27, 2012 were filed in Clark County Probate Court; and the consent hearing on said petitions will be had before said Court on September 2, 2020 at 10:00 A.M. followed by a best interest hearing to be held on September 29, 2020 at 10:00 A.M. It is alleged in the Petitions for Adoption, pursuant to R.C. 3107.07, that the consent of Brooke Jacks fka Brooke Elaine Sparks is not required due to the following: (X) the parent has failed without justifiable cause to provide more than de minimis contact with the minor for a period of at least one year immediately preceding the filing of the adoption petition or placement of the minor in the home of the petitioner. (X) the parent has failed without justifiable cause to provide for the maintenance and support of the minor as required by law or judicial decree for a period of at least one year immediately preceding the filing of the adoption petition or the placement of the minor in the home of the petitioner.
