Case No. 19-CRF-0069 On May 9, 2019, the Ohio Medical Board mailed a Notice of Opportunity for Hearing to Ashley Rae Strunk, L.M.T., via certified mail, return receipt requested, at her last known address of record, 5405 Camelot Drive, Apt. A, Fairfield, OH 45014-7458. The Notice was returned to the Board from the postal service marked "unable to forward." A copy of the Notice is available on the Board's website at www.elicense.ohio.gov. The notice states that the Ohio Medical Board intends to consider disciplinary action against Ms. Strunk's certificate to practice as a massage therapist in Ohio based on her failure to cooperate in a Board investigation. The foregoing would constitute a violation of Section 4731.22(B)(34), Ohio Revised Code. Ms. Strunk is entitled to a hearing in this matter if such hearing is requested within thirty (30) days of the last date of publication of this notice. Ms. Strunk may appear at such hearing in person, by her attorney, or by such other representative permitted to practice before this agency, or she may present her position, arguments or contentions in writing.
